Physicians’ Care Clinic acts as a ‘safety net’ for vulnerable DeKalb County residents in need of non-emergent primary medical care. We are the oldest and largest free clinic in DeKalb County.

Check Your Eligibility

Patients of Physicians’ Care Clinic must meet the following conditions:

  • Be a DeKalb County resident

  • Be uninsured

  • Be ineligible for Medicare or Medicaid coverage

  • Income must fall below the 200th percentile of the federal poverty level

The above criteria are designed to ensure the clinic is able to provide services to DeKalb County residents most in need of care, and those who otherwise would not be able to access the medical services they need.

Fill Out Application

To become a patient at Physicians’ Care Clinic, the first step is to complete the application below. You will need to submit the following:

  • A valid picture ID (driver license, visa, passport, green card, state-issued ID card)

  • Proof of residency (rental lease, utility bill showing current address, notarized letter from landlord)

  • Proof of income (A wage inquiry from the Georgia Department of Labor with 1 month of check stubs; or if check stubs are not available, a statement of gross amount received from your employer on company letterhead, if you receive Supplemental Security Income (SSI), an award letter with monthly amount is required.)

Free Services for Uninsured Men

In 2018, the Fulton-DeKalb Hospital Authority awarded the Dekalb County Board of Health a grant to launch the Men’s Clinic program. The goal of the clinic is to promote health services throughout the county and encourage more men to receive regular screenings and address health concerns.
